Creating a Poll or Survey

Adding The Poll Field

You can add as many Poll Fields as you want. Each Poll Field represents a single question, so if you have 10 questions, then you would add 10 Poll Fields. 

To add a Poll Field, do the following:

  1. With your Form open in the Form Edit Screen, expand the Advanced Fields options.
  2. Drag & Drop the Poll fields from the options into your Form, where you would like your question to appear. You can move it later if needed.
  3. Click on the new Untitled Poll Field in your form or click the triangle icon to open the configuration options.

General Configuration

Configure your Poll Question as follows:

  • Description – Enter the description for the form field. This will be displayed to the user and provide some direction on how the field should be filled out or selected.
  • Poll Question – Enter the question you would like to ask the user. The user can then answer the question by selecting from the available choices.
  • Poll Type – Select the field type you’d like to use for the Poll. Choose radio buttons or dropdown if question only has one correct answer. Choose checkboxes if your question requires more than one correct choice.
  • Field Choices – Add Choices to this field. You can mark each choice as checked by default by using the radio/checkbox fields on the left. Click the “+”  or “-” icons to add or remove more choices.
  • Bulk Add / Predefined Choices – Select a category and customize the predefined choices or paste your own list to bulk add choices.
  • Enable “Other” Choices – Check this option to add a text input as the final choice of your radio button field. This allows the user to specify a value that is not a predefined choice.
  • Randomize Order of Choices – Check the box to randomize the order in which the choices are displayed to the user. This setting affects only voting – it will not affect the order of the results.
  • Required Field – Select this option to make the form field required. A required field will prevent the form from being submitted if it is not filled out or selected.

Add as many Poll Questions as you want. Each Question can be differently configured and total scoring is based on all combined questions  in your Form.

Save Your Form – Once you’ve finished building your form, click on the Update button to save your work. You can always edit the form later.

Adding the Survey Field

You can add as many Survey Fields as you want. Each Survey Field represents a single question, so if you have 10 questions, then you would add 10 Survey Fields. 

To add a Survey Field, do the following:

  1. With your Form open in the Form Edit Screen, expand the Advanced Fields options.
  2. Drag & Drop the Survey fields from the options into your Form, where you would like your question to appear. You can move it later if needed.
  3. Click on the new Untitled Survey Field in your form or click the triangle icon to open the configuration options.

General Configuration

Configure your Survey Question as follows:

  • Description – Enter the description for the form field. This will be displayed to the user and provide some direction on how the field should be filled out or selected.
  • Survey Question – Enter the question you would like to ask the user. The user can then answer the question by selecting from the available choices.
  • Survey Field Type – Select the type of field that will be used for the survey. Choose from: Likert, Rank, Rating, Radio Buttons, Checkboxes, Single Line Text, Paragraph Text, Drop Down

Survey Field Types

Likert Configuration

  • Likert Columns – Enables a single answer from an array of choices.
  • Enable Scoring – Allows different scores for each column. Aggregate scores are displayed in the results page and can be used in Merge Tags.
  • Enable Multiple Rows – Select to add more rows to your Survey Question
  • Required Field – Select this option to make the form field required. A required field will prevent the form from being submitted if it is not filled out or selected.

Rank Configuration

  • Ranking allows the user to drag & drop to shuffle and reorder the answers to the question
  • Field Choices – Add Choices to this field. You can mark each choice as checked by default by using the radio/checkbox fields on the left.

Rating Configuration

  • Rating lets the user give the question “star” ratings; usually the more, the better.

Radio Buttons

  • Users answer the question with only a single answer, by clicking a button

Checkboxes

  • Users may select multiple answers with Checkboxes

Single Line Text

  • Users enter an answer in a text field. 
  • Create Content Template – Insert Merge Tags from the dropdown selections
  • Input Mask –  provides a visual guide allowing users to more easily enter data in a specific format such as dates and phone numbers.

Paragraph Text

  • Users enter an answer in a paragraph text field (multiple lines)
  • Maximum Characters – Enter the maximum number of characters that this field is allowed to have.

Drop Down

  • Users answer the question with only a single answer,selected  from the drop down list

 

Required Field – Select this option to make the form field required. A required field will prevent the form from being submitted if it is not filled out or selected.

No Duplicates – Select this option to limit user input to unique values only. This will require that a value entered in a field does not currently exist in the entry database for that field.

Add as many Survey Questions as you want. Each Question can be differently configured and total scoring is based on all combined questions  in your Form.

Save Your Form – Once you’ve finished building your form, click on the Update button to save your work. You can always edit the form later.
